Fading libido and softer erections in your forties are almost never a testosterone story first. They are a blood flow story. And blood flow is the earliest, cheapest, most ignored cardiovascular warning system a man owns.
A man in his mid-forties sits across from me and tells me his drive is gone. Performance is inconsistent. He has already done what most men do: he searched his symptoms, found a clinic, and got a testosterone panel. The number came back low-normal. Someone offered him a prescription.
Nobody asked him about his arteries.
That is the part that should bother you. Because in a meaningful number of these men, the bedroom complaint is not the problem. It is the receipt. It is the first place a systemic vascular problem becomes loud enough to notice, and it typically shows up years before anything happens to the heart.
The smallest pipe fails first
This is not a wellness theory. It is a documented model in cardiology called the artery size hypothesis, and it is almost embarrassingly simple.
Atherosclerosis and endothelial dysfunction do not politely confine themselves to one region. They are systemic. They develop across your entire vascular tree at a broadly similar pace. So the first vessel to become symptomatic is not the most important vessel. It is the narrowest one.
Where symptoms surface first
Approximate vessel diameter. The same degree of plaque burden or endothelial dysfunction becomes functionally obvious in the smallest channel long before the larger ones complain.
The clinical consequence: across the longitudinal literature, erectile difficulty in men with no known cardiovascular disease tends to precede a cardiovascular event by roughly two to five years. Pooled analyses covering hundreds of thousands of men put the odds of cardiovascular disease meaningfully higher in men with erectile dysfunction, with endothelial dysfunction identified as the shared mechanism underneath both.
You were not handed a sexual problem. You were handed a five-year head start on a cardiac one. Most men trade that head start for a prescription and learn nothing.
What actually produces an erection
Here is the piece of physiology that reframes everything, and it is the reason the testosterone-first approach so often disappoints.
An erection is not a contraction. It is a relaxation.
Smooth muscle in the arteries and the spongy erectile tissue has to let go. When it relaxes, blood floods in, the expanding tissue compresses the outflow veins against the surrounding sheath, and the pressure holds. The signal that tells that smooth muscle to release is nitric oxide, converted downstream into cyclic GMP.
That is the entire mechanism. And it is worth noticing that the most famous erectile dysfunction drug in the world does not touch testosterone at all. It protects cyclic GMP from being broken down, so a nitric oxide signal that was already too weak to finish the job gets to stay in the room longer. The drug is a workaround for a signaling deficit. It never asks why the signal got weak.
Nitric oxide is also not a bedroom molecule. It regulates vascular tone, blood pressure, platelet behavior, and oxygen delivery to every tissue you have. Erectile tissue is simply where a shortage announces itself first, for the reason shown above.
The testosterone question, answered honestly
You will see this framed online as a fight: it is nitric oxide, not testosterone. That framing is wrong, and I want to be precise about why, because both camps are selling something.
Testosterone does not mechanically dilate the vessel. That much is true. But testosterone helps build and maintain the machinery that does. Androgens regulate the expression and activity of nitric oxide synthase in erectile tissue. Testosterone deficiency is associated with reduced nitric oxide availability, partly through effects on that enzyme and partly through elevated ADMA, which is an endogenous brake on nitric oxide production. It is one documented reason PDE5 medications underperform in testosterone-deficient men.
So the correct statement is not either-or. Low testosterone degrades the nitric oxide system. And the things that drive testosterone down in modern men, insulin resistance, visceral fat, chronic inflammation, poor sleep, poor cardiorespiratory fitness, are the same things that wreck endothelial function directly.
Two symptoms, one upstream cause. Chase either number in isolation and you will spend money treating a readout instead of a system.
On aging: nitric oxide capacity genuinely declines with age. You will see dramatic figures thrown around, and I am not going to repeat the biggest ones because I cannot trace them to solid human data. The defensible version is that endothelial function declines on the order of ten to twelve percent per decade beginning far earlier than most men assume, and how fast it declines is heavily modifiable. That is the honest number, and it is still alarming enough.
Before you supplement, get information
If your sexual function has changed, especially before sixty, the responsible first move is not a powder. It is data. Ask a physician for a real vascular workup rather than accepting a lone testosterone value as the explanation.
What belongs in that conversation
- Coronary artery calcium scoring, to look for calcified plaque you cannot feel
- Carotid ultrasound, a non-invasive read on arterial wall health
- Fasting insulin, not just fasting glucose, plus HbA1c and a full lipid panel with ApoB
- Inflammatory and vascular markers including hs-CRP and homocysteine
- A sleep apnea screen, which is one of the most commonly missed drivers in this entire picture
- A complete hormone panel including free testosterone, SHBG, estradiol and thyroid, interpreted in context rather than in isolation
One useful clue you can observe without a lab: if you still wake with morning or overnight erections, the hardware and the plumbing are likely intact, which shifts the conversation toward stress, sleep, medication effects, relationship factors and recovery. If those have disappeared, the vascular workup becomes more urgent, not less.
The free levers come first
I have watched this order of operations decide outcomes for thirty-three years. Supplements layered onto a broken foundation are expensive disappointment. So we earn the foundation first.
- Load the biggest muscles you own. Squats, deadlifts, and heavy carries recruit enormous muscle mass, which drives blood volume and shear stress through the vessel wall. Shear stress is the primary physiologic stimulus for endothelial nitric oxide production. This is the closest thing to a free prescription in the entire discussion. Build to it with real technique. Nobody needs a hero deadlift at forty-eight.
- Eat the nitrate pathway, do not just talk about it. Leafy greens, arugula, beets and beet root powder supply inorganic nitrate that oral bacteria convert to nitrite and then to nitric oxide. This route becomes more valuable as you age, because it works when the enzymatic pathway is struggling.
- Stop killing the bacteria that do the conversion. Antiseptic mouthwash disrupts the oral nitrate-reducing bacteria that pathway depends on. Small habit, real effect.
- Fix glucose control. Chronically elevated glucose and insulin degrade endothelial function directly. Here I will part company with the popular version of this advice: the reason lower-carbohydrate eating helps is glycemic control, not some direct nitric oxide effect, and fasting is a tool for the same job rather than a nitric oxide strategy in its own right. Do not skip the greens because someone told you fasting handles it.
- Sleep, stop nicotine in every form, and train aerobically. Unsexy, non-negotiable, and more decisive than any bottle on this page.

Here is a piece of honesty you will not get from most supplement companies, including about their own products. The most cited human trial on citrulline and erection hardness used one and a half grams of pure L-citrulline daily and moved half the men in the mild category back to normal hardness, against roughly eight percent on placebo.
